School Nurse/ Medication Information
Our school nurse is Erica Montegna.
Students see the school nurse for anything from a tummy ache or bandaid to a fever. The nurse will use her discretion in notifying the parents of a student’s visit to her office.
The School Nurse has access to all students' medical information as it was provided by parents in the Infosnap Registration Form. To ensure the best care and safety of your child, the school should have up-to-date information on ANY prescribed medication your child is taking (taken at home or at school).
If prescriptions change after completing the registration form, please email the nurse and your child’s teacher.
All medication information is logged into PowerSchool (our student information database). Medication that requires a dose during school hours must be provided to the school by a parent, in the original container. No medication can be dispensed without filling out the OCS Medication Authorization Form. This medication will be held in the nurse’s office and administered by the nurse or a member of the administration in the absence of the nurse.
An asthma action plan/FARE allergy action plan must be turned in.
Please contact the nurse directly at 773.348.8212 or by email if your child has been diagnosed with a significant illness (Strep, Chicken Pox, Pneumonia, COVID-19, etc).
All medical forms needed to be turned in to our nurse by June 1st. You can email these to emontegna@stclementschool.
If your child's exam for the Health and Immunization form cannot be scheduled until after June 1st, you must submit a letter from their doctor with the date of appointment and a list of immunizations up to the current date. There will be no exceptions.

State Vision Examination Form
Kindergarten and all out-of-state students entering Saint Clement must turn in the attached document for the eye examination.
State Dental Form
Students entering K, 2nd, and 6th-grade turn in the attached form for dental examination.
